A general psychiatric provider may be very good at treating anxiety, depression, bipolar disorder, or other mental health conditions. A provider with advanced training in perinatal and reproductive mental health brings another layer of expertise: understanding how those symptoms can change during pregnancy, after birth, around fertility challenges, or during hormonal transitions.

What Does Perinatal Psychiatry Actually Cover? 

Much more than postpartum depression. 

Perinatal mental health generally includes pregnancy and the first year postpartum, but reproductive psychiatry looks more broadly at how hormonal and biological factors can affect emotional well-being throughout the reproductive lifespan. 

Someone might seek care because they’re experiencing prenatal or postpartum anxiety. Someone else may need help managing bipolar disorder during pregnancy. Another person may be struggling after a pregnancy loss, going through fertility treatment, or wondering why their mood suddenly feels completely different during perimenopause.

Perinatal mental health concerns are treatable, and care may include psychiatric medication, therapy, or both. The right treatment plan depends on the person, their symptoms, health history, goals, and stage of life. 

So, What’s the Difference Between a Specialist and a General Psychiatric Provider? 

Think of it less as “one is good and one isn’t” and more as depth of experience. 

A general psychiatric provider may see a handful of pregnant or postpartum patients. A perinatal mental health specialist spends much more time thinking about the questions that come with these stages of life. 

For example: Should someone continue a psychiatric medication during pregnancy? What changes if they plan to breastfeed? Could new anxiety actually be postpartum OCD? How does severe sleep disruption affect someone with bipolar disorder? Is a sudden low mood connected to depression, hormones, or both? 

Those are layered questions. 

Current guidance from the American College of Obstetricians and Gynecologists recognizes that mental health treatment during pregnancy and postpartum may include psychiatric medication management and recommends against automatically stopping medication solely because someone is pregnant or breastfeeding. Decisions should weigh the risks and benefits for the individual. 

That kind of collaborative decision-making is a big part of reproductive psychiatry. 

Why Does Reproductive Mental Health Need Specific Training? 

Because your brain doesn’t exist separately from the rest of your body. 

Hormonal shifts can affect sleep, mood, energy, anxiety, concentration, and emotional regulation. Pregnancy and postpartum also bring physical recovery, feeding decisions, relationship changes, disrupted sleep, and, let’s be fair, a truly impressive amount of unsolicited advice. 

A specialist understands how those pieces can overlap. 

They’re also trained to notice when symptoms may need a closer look. ACOG recommends screening for depression and anxiety during pregnancy and postpartum and considering bipolar disorder when evaluating certain mood symptoms. 

Good perinatal psychiatric care may also involve collaboration with your OB/GYN, nurse midwife, therapist, primary care doctor, or other healthcare providers. Mental health treatment works better when everyone involved understands the plan.

You can have highly specialized psychiatric care and still talk to someone who feels like a real person. 

You should be able to ask whether a medication is safe, say you’re nervous about treatment, admit that you’re overwhelmed, or tell your provider that the current plan simply isn’t working. That’s the kind of perinatal psychiatry UpWell is trying to provide across Oregon and Washington: clinically thoughtful, trauma-informed, collaborative, and still human. 

Because reproductive mental health can be complicated, but getting support for it shouldn’t have to be.
