The Site is intended for adults 18 years of age and older who are located in the United States. Clinical services are available only to individuals physically located in Oregon or Washington at the time of their appointment, consistent with state licensure requirements.

Information you submit through website forms or unencrypted email is not guaranteed to be secure and should not include detailed clinical information. Please use the patient portal for protected health information once you are an established patient.
By providing your phone number or email address, you consent to receive communications from UpWell Psychiatry related to your inquiry, appointment, or care. Message and data rates may apply. You may opt out of non-essential communications at any time.
Our collection and use of personal information is described in our Privacy Policy, which is incorporated into these Terms of Use by reference.

To the fullest extent permitted by law, our total liability for any claim related to the Site will not exceed one hundred dollars.
