Patient Portal

Current Patients: We use the CHARM Patient Health Portal for all communication.

In the portal you can: Message your provider, Schedule new appointment, Cancel an appointment, Request refills and more. Below, you can download the Charm mPHR app from the Apple or Google store for mobile use, or you access from your browser.

For Billing Questions: Please message our practice administrator, Kristine Deytiquez, in the Charm Portal.

To join an appointment: We use ZOOM rooms for our telehealth meetings. Your provider has a stable link that never changes. Please check your email to find the link for your provider, or email our office.
